It can be genuinely difficult to distinguish ordinary age-related changes from something worth investigating, which is exactly why it’s worth mentioning to a doctor rather than assuming.

Urinary Symptoms

Frequent urination, waking at night to urinate, a weak stream, difficulty starting, or dribbling afterward are commonly associated with prostate issues. Most of the time, these symptoms trace back to the prostate simply enlarging with age (benign prostatic hyperplasia) rather than cancer — though cancer can cause these symptoms too, particularly if it grows toward the urethra and narrows it.

The Most Common Symptom: No Symptoms at All

This is genuinely the most common way prostate cancer presents. It makes sense once you understand where cancer tends to start: often in a quiet region of the prostate gland, away from the urethra and other structures. Cancer growing there doesn’t press on anything, so it causes no pain and no urinary symptoms. If it grows toward the urethra, it can cause urinary narrowing; if it spreads beyond the gland, it causes symptoms wherever it lands — like bone pain. But early on, in its original location, it’s frequently silent. The absence of symptoms simply doesn’t mean the absence of disease.
