Nightly: Mentholated ointment under a sock. A 2011 study at a U.S. Air Force medical center had 18 people with lab-confirmed nail fungus apply an over-the-counter mentholated chest rub (containing thymol, camphor, eucalyptus oil, and menthol) once daily for 48 weeks. At the end, 27.8% had a complete, lab-confirmed cure, and another 55.6% had partial clearance — meaning 83% of participants showed some positive effect. For comparison, a commonly used prescription antifungal is generally reported around a 34% cure rate in other research. This was a small, 18-person pilot study with no placebo group, a real limitation worth keeping in mind. Apply after showering and drying the foot completely, working the ointment into the nail, surrounding skin, and under the front edge, then cover with a clean cotton sock to hold it in place overnight.
Morning: Tea tree oil. A 1994 double-blind, randomized controlled trial gave 117 patients with lab-confirmed nail fungus either 100% tea tree oil or a 1% comparison solution twice daily for 6 months. Roughly 60% of both groups showed partial to full improvement, with complete cure rates of 18% for tea tree oil compared to 11% for the pharmaceutical comparison. A separate, smaller open study (20 patients, no control group) combining vitamin E with lime, oregano, and tea tree essential oils found a 78.5% complete cure rate with no reported side effects — a promising but much less rigorously controlled result, worth treating as preliminary.
Optional: Ozonated sunflower oil. A randomized, open-label phase 3 trial in Cuba compared topical ozonated sunflower oil against a 2% ketoconazole cream in 400 patients over three months. The ozonated oil group showed a 90.5% cure rate compared to 13.5% in the ketoconazole group. This is a striking result, but it comes from a single open-label study on the researchers’ own product that hasn’t been independently replicated at scale outside that research group — worth treating as an interesting but unconfirmed finding rather than an established standard.
If layering treatments, apply one product at night and a different one in the morning rather than combining everything at once, and stay consistent through a full nail growth cycle rather than switching frequently.
Supporting the New Nail While You Treat the Old One
Since killing the fungus is only half the process, and your body has to grow an entirely new, healthy nail underneath, adequate protein intake matters during this period, since nails are built from protein. One human study found that people with brittle nails who took collagen peptides daily for 24 weeks saw a 12% increase in nail growth rate, a 42% drop in broken nails, and improvement in nails for 64% of participants. Collagen doesn’t treat the fungal infection itself — its role here is simply supporting the raw materials for healthy nail growth alongside whatever antifungal approach you’re using.
Closing the Loop: Environmental Prevention
Treatment alone often fails without addressing the environment around it:
- Wash socks at 140°F (60°C) or higher — a 2022 disinfection trial found that washing at 104°F (40°C), steam washing at the same temperature, and freezing all failed to kill the fungus; only higher-temperature washing worked.
- Keep contaminated socks separate from clean laundry rather than letting them sit together in a hamper, given the documented cross-contamination risk.
- Rotate shoes and give each pair 24-48 hours to fully dry between wears.
- Monitor blood sugar — aim for a fasting glucose in the 70-90 mg/dL range and an A1C at or below 5.2% if you’re tracking these, given the glycation mechanism discussed earlier.
